HIV/AIDS Case Management services are client-centered
activities through which services for persons living
with HIV/AIDS are coordinated toward improving health
outcomes and facilitating client self-sufficiency.
The case manager acts as a liaison and advocates for
clients to access service providers while educating
the client on relevant information and available resources.
Case manager assess and monitor the progress of the
client on an on-going basis. They identify and address
needs in areas relating to the client’s medical,
psychosocial, environmental, financial, health education
and risk behaviors.
The primary goal of HIV/AIDS Case Management is to
increase client self-sufficiency by providing quality
services in the most efficient and effective manner
possible to individuals with multiple and complex needs.
Case manager strive to:
• Ensure, that to the fullest extent possible,
clients receive and maintain access to appropriate
services to promote maximum quality of life, independence,
and self-sufficiency.
•
Increase the access and the utilization of medical
and social services by eliminating barriers and promoting
early and on-going intervention and adherence.
•
Increase access to HIV information and education as
well as to foster harm-reduction activities that address
high-risk behaviors.
•
Focus on improving the health outcomes of person living
with HIV/AIDS by encouraging the family and social
support system to assist the client in adhering to
primary medical care.
Who is eligible for case management services?
HIV/AIDS Case Management services shall provide to
indigent person living with HIV/AIDS residing within
Los Angeles County. Our target population is Asian
Pacific Islander those infected with HIV/AIDS. To be
an eligible client, a diagnosis form completed by physician
and proof of Los Angeles County residency is needed.
